QUESTIONNAIRE

The questionnaire provides information about the author and co-authors. Pp. 1-5 must be completed in Ukrainian and English. Sample:

1. Surname, name of the author / authors.*
2. Academic title.*
3. Scientific degree.*
4. Place of work, position. Indicate the full name of the institution, department or other structural unit without abbreviations.*
5. Work address. Specify postal code, country, region \ district, city \ locality, street, building number.*
6. Work phone and email. If you do not have a personal work phone and email, you must specify another method of online communication.
7. Home address: specify postal code, country, region \ district, city \ locality, street, house number, home and mobile phones, email.
8. Field of research and research interests: indicate the research topic or field of science being studied.
9. Information about the author: indicate the personal website of the author or profile on social networks (subject to availability and optional).

STATEMENT

The statement that the article is the author's own development (authors), has not been published anywhere before and is not under consideration in other publications, the authors give written consent to publish the material and their personal data in the Bulletin of Kharkiv State Academy of Design and Arts.

I am Ivan Ivanov, certify that the article submitted for publication in the "Bulletin of the Kharkiv State Academy of Design and Arts" (hereinafter – the Bulletin), on "Peculiarities of Environmental Design in Kyiv", 12 pages, is my / our own development, has not previously been published or published in other scientific journals, is not under consideration in them.

I have read the requirements for the submission and design of scientific articles for publication and agree to publish the article in the next issue of the Bulletin in accordance with the order determined by the Editorial board. We also consent to the use of the article in electronic databases, which includes the Bulletin.
